Questionnaires are the fastest way to ask a client a focused set of questions - think pre-event details, preferences, or follow-up info - and track exactly where each one stands, from Draft all the way to Completed. Clients answer through a public link, no login needed.
The Questionnaires page
Open Questionnaires from the main menu. Every questionnaire in your account is listed here, and you can:
- Filter by status using the pills at the top: All, Draft, Sent, Viewed, Answered, and Completed. The status updates as your client opens and answers the questionnaire, so you always know who needs a nudge.
- Create a new one: give it a name and, optionally, link it to a booking and a client. Linking matters - it connects the answers to the right event and person automatically.
- Open the editor for any row to work on its questions.
- Delete a questionnaire you no longer need.

Build your questions in the editor
- Open a questionnaire to enter the Questionnaire Editor.
- Add a question inline: type the question text and pick a type - Short text, Long text, Single choice, Multiple choice, Date, Yes / No, or Scale 1-10.
- For Single choice and Multiple choice questions, add the answer options your client picks from.
- Toggle Required on any question the client must answer before submitting.
- To change the order, grab a question's drag handle and drop it where you want it. The new order saves immediately - there's no separate save step.
- Use the delete action on a question to remove it, and Back to return to the list.
You can come back and edit questions at any point. Changes apply to the questionnaire your client sees at the public link.
Share it with your client
When the questions are ready, send the questionnaire from the list page. If the linked client has an email address on file, KM Hub emails them the fill link directly and marks the questionnaire as Sent. If there's no email on file, you'll get the public link to share manually - it looks like /questionnaire/... and works in any browser, no account required. You can also use Copy link on any questionnaire to grab that public URL and paste it into a text message or your own email.
What your client sees
The public fill page shows your questions one clean form at a time. As the client opens it and submits answers, the status on your Questionnaires page moves from Sent to Viewed to Answered, so you can filter for anything still waiting and follow up before the event.
Questionnaires vs. planning forms
Use a questionnaire for a quick, one-off set of questions tied to a specific booking or client. Use a planning form (built in Planning Forms) when you want a reusable, branded, multi-section form you send for every event of a certain type.
